Checklist for Placement
Schools may use this as general guidance of interventions or process that should take place before a placement in EBIT is considered.
- Student has been discussed at S-Team/Behavior Team meeting and a plan has been discussed to observe behavior of student in classroom and special classes setting by Behavior Specialist.
- Referral documentation is attached.
- EBIT program teachers have been informed and will participate in the observation and FBA creation and implementation along with the Behavior Specialist and school Psychologist.
- A functional behavior assessment has been conducted by Behavior Specialist.
- IEP has been reviewed and considered when determining behavioral needs.
- A behavior plan has been created and interventions are being implemented and monitored.
- Teacher and support team (including EBIT program teachers) have met with parents to discuss concerns and all parties are involved in creation of behavior plan.
- Behavior team will meet periodically (minimum of every 30 days) to discuss progress as well as adjusting behavior plan to meet student needs.
- Interventions must be monitored over time and recorded on data sheet.
- Progress monitoring data must show that progress has NOT been made and therefore alternate placement decision must be implemented.
- Meeting must be held with all members of behavior team and parents to determine if EBIT is an appropriate placement.
